Prepare for your appointment to make the most of your time with your doctor.

Before Your Visit

  • Bring your INR log or tracking records
  • Bring a complete list of all medications and supplements
  • Write down any symptoms or concerns you want to discuss
  • Note any missed warfarin doses since your last visit
  • Bring this sheet to take notes during the appointment
  • Know your current warfarin dose and schedule
  • List any recent changes to your diet or lifestyle

Things to Report

  • Any new medications or supplements (including OTC)
  • Missed or extra warfarin doses
  • Unusual bleeding or bruising
  • Changes in diet (especially vitamin K-rich foods)
  • New diagnoses or medical procedures
  • Planned dental work or surgeries
  • Pregnancy or plans to become pregnant
  • Changes in alcohol consumption
  • Recent illness (fever, vomiting, diarrhea)
  • Falls or injuries, even if they seemed minor

Questions to Ask Your Doctor

  • What is my current target INR range?
  • How often should I be getting my INR tested?
  • Are there any changes to my warfarin dose?
  • Are any of my other medications interacting with warfarin?
  • Is it safe for me to take [specific OTC medication]?
  • Should I be concerned about my vitamin K intake?
  • When should I schedule my next INR test?
  • What should I do if I miss a dose?
  • Are there any new warnings or guidelines I should know about?
  • Should I consider switching to a different anticoagulant?
